The zemsky vrach (zemstvo, or district, physician) was the salaried doctor employed by the zemstvo board to serve an entire rural district -- often single-handedly responsible for public health across dozens of villages, which explains Radushny's later exasperation at being buried in correspondence rather than left to treat patients.
